Health Risk Assessment and Biometric Screenings

The City of La Crosse offers annual Health Risk Assessment (HRA) questionnaires and biometric screenings to employees, retirees and spouses enrolled in the City's medical benefit plan and to non-represented employees (and their spouses) that are eligible for but not enrolled in the plan. This confidential preventive care screening is included in your benefit package at no cost to you.

Screenings are used as a tool to provide you with a snapshot of your well-being, while identifying risks for certain chronic diseases. Tests include:

  • Complete Metabolic Panel (consisting of 14 different tests)
  • TSH (Thyroid)
  • Lipid Screening 
  • Blood Pressure, Height and Weight
  • Optional Screening for Prostate Cancer (PSA) for Males Over Age 50

Employees and retirees who complete the HRA/biometric screening process receive a participation incentive of a lowered monthly premium contribution towards their medical benefit plan, beginning January 1 of the following year. New employees who enroll in the City's medical benefit plan automatically receive the lowered premium until offered such opportunity to participate.

Your Health Risk Report is personal and confidential. The City of La Crosse does not receive individual results, only a de-identified aggregate report to assist in designing wellness programs. Your name and participation status may be disclosed to the City for the purpose of administering the lowered monthly medical benefit plan contribution rate.

Gym Membership Reimbursement Program

Gym membership reimbursement is available to regular full-time employees enrolled in the City's medical benefit plan.

Employees may be reimbursed up to $300 per calendar year for eligible gym membership fees. Eligible fees include annual or monthly fees for an individual membership at one fitness center or studio.

Employees must complete a minimum of twelve (12) workouts or classes per calendar month to receive reimbursement.

La Crosse 1856 Wellness Program

Whether your focus is on physical, mental, or emotional well-being, the La Crosse 1856 program provides a variety of resources, challenges, and activities aimed at guiding you on your path to a healthier, more balanced life. Named after the year the City was incorporated, La Crosse 1856 gives employees an opportunity to earn incentives and prizes by reaching 1,856 points through activity, awareness, healthy choices, and prevention.

This voluntary wellness program is available to the following full- and permanent part-time employees at no cost:

  • ATU Local #519
  • Non-Represented
  • Police LPPNSA and Police LPPSA

The program runs January 1 - November 30 and there is no deadline to participate. The goal is to earn 1,856 points or more by November 30. Participants can earn prizes and incentives throughout the program.

Reusable Container Program

Employee Wellness partners with Western Technical College's Union Market for a reusable container program. City employees can purchase a set of reusable food containers for $5 to be used at Western Technical College's Union Market. Employees can save anywhere from 69¢ to $2 on their meal when using the containers and electing healthier options.

The Union Market is located on Western Technical College's main campus, located on the first floor of the Kumm Center at 411 7th St. N.

Employee Assistance Program

The Employee Assistance Program (EAP) is available at no cost to all benefits‑eligible full‑ and part‑time City employees, as well as their legal spouse and dependents ages 12 and older. This program provides professional, confidential support for personal or work‑related concerns. Employees and their immediate family members may receive up to six (6) sessions per issue at no cost.

EAP services are available 24 hours a day, seven days a week. All information shared is confidential and is not provided to your health care provider or employer.
