Claim forms must be as complete and legible as possible and filed with the City Clerk's Office.
All claims are referred to the City's insurance provider. The City Clerk's Office cannot provide legal advice. For questions about pending claims, please contact Wisconsin Municipal Mutual Insurance Company (WMMIC) at 608-246-3336.
